There is a universal risk for medication safety issues in older people in need of care and services in hospital, residential aged care and at home. The 10 minute Look and Act toolkits optimised for mobile phones or other devices are designed as ‘on the job resources’ to identify and respond to the risk that often has serious and sometimes fatal consequences. Medication safety is a high impact/high prevalence risk under Standard 3 of the Australian Aged Care Quality Standards and requires prompt recognition and management.

Audience

Anyone providing care and services to older people in residential aged care, acute care, general practice and in the community.
