Course Overview
This course forms part of our suite of online courses developed to support the implementation of part one of the Care Act 2014 and based on Skills for Care training materials. It will give you all the insights you need to help young people during this challenging time.
You’ll learn about how culture can impact on the process of moving from childhood into adulthood, tips for transitioning into adulthood and the specific challenges of transition to adulthood for youths with disabilities.
In particular, it covers the educational, healthcare and social support needs that children moving into adulthood may require – and explains how those needs change dramatically across the period of a few months or years.
You’ll get a certificate on course completion, complete with your name, CPD hours, today’s date and the learning objectives covered. You’ll also have access to additional resource materials, useful links and handy refresher guides for future reference."
Target Audience
This course is ideal for you if you’re a practitioner directly involved in the planning for and support of young people, including those with special educational needs (SEN) and complex needs – as well as their carers.
It’s also useful for specialist information and advice workers and their managers as well as managers and commissioners in local authorities with a responsibility for the provision and market development of services for young people who will transition to adult care and support."
